Beyond Well Fee-For-Service Terms

I understand that I have chosen not to enroll in The Method membership program at this time and elect to continue receiving care through Beyond Well on a fee-for-service basis.

I acknowledge and agree to the following:

• I will be charged for each office visit, telehealth visit, procedure, or other service provided.

• Follow-up appointments, including in-person and telehealth visits, will be billed at the current rates published on Beyond Well's appointment booking page at the time of service.

• Fees for laboratory testing, compounded medications, supplements, and other products or services are based on current market pricing and are subject to change without notice.

• Payment is due at the time services are rendered unless otherwise agreed upon by Beyond Well.

• I understand that by declining membership, I am also declining the benefits available exclusively to members, including priority scheduling, member pricing, and other membership-exclusive benefits.

• Appointments will be scheduled based on general appointment availability and are not guaranteed within a specific timeframe.

• I understand that I may choose to enroll in The Method membership at any time, subject to program availability and the membership terms and pricing in effect at the time of enrollment.

• This Fee-for-Service Agreement will remain in effect until I notify Beyond Well in writing that I wish to enroll in The Method or until this agreement is otherwise updated or replaced.
